Hi Sir,

I have applied on 08th Jan''18 to make the correction in my name as previously my name was chandan kumar ray but now I need only chandan kumar as in my all academic and aadhar mentioned only chandan kumar, I have applied online and paid 110.00, attached with required document I sent in mumbai UTI office.

Today I have checked online, its showing we have not received your document. I have acknowledgement from my Currier company that it has already been dispatch. please check at your end and let me know the exact status.
