There are two thing; PAN Number and PAN Card.
PAN Number doesn't change even if you lost your PAN Card. It is a unique number which is issued one time. By law, an individual cannot have 2 PAN Number. This could result in to penalty of Rs 10,000 along with other legal actions.
If you have lost you 'PAN Card' then yes you can get another one by submitting the'Change Request Form' which you can download from the official website of NSDL PAN Service or UTI.
The detail instruction for filling the application and list of document to attach are available on application form.
Note that you need to submit; ID Proof, Address Proof and DOB Proof with your Photo on the application form.
