Kindly re-submit colour Photograph (3.5 cm *2.5 cm) of Minor along with Representative Assessee signature across the photograph on a separate paper.

You have to stick a photograph of minor on blank page and tell your representative assess ( Parents or guardian whomever you have taken in application ) to sign across the photograph and submit it to NSDL .
